The TRU PACE program provides older adults the care they need to continue living safely in their community while also maintaining their independence. This program is a managed medical plan that helps older adults meet their health care needs in their home and in the community.

How TRU PACE Helps Older Adults in Boulder County

TRU PACE helps older adults navigate the complicated issues associated with aging. Our program helps older adults to live in their own home or in a community supported facility. At our Day Center, Participants can meet with a provider or schedule an appointment at our Clinic, participate in therapy sessions, and speak with their Case Manager or Social Work team

Participants also receive support at home, transportation to and from the day center as well as medical appointments when it is part of your coordinated care plan. Participants receive support all in one place. Your team explains what is in your plan and why it helps you stay safe at home.

How to Get Started

Start with a short call with our Intake Specialist.  They will verify your location is within our service area, you meet the qualifications for TRU PACE and discuss with you how TRU PACE can meet your needs so you can remain independent and continue to live safely in your community.
