Enrollment Process
Step 1
When a potential participant, family member, or referral source contacts TRU PACE (Program of All-Inclusive Care for the Elderly), our Intake Specialist gathers basic information to understand the individual’s needs and explain how the program works.
During the conversation, we discuss health concerns, memory or mental health needs, daily support at home, and any gaps in care. This helps determine whether PACE may be a good option and allows us to share the services available through TRU PACE.
We may also discuss general financial information to help identify whether the individual may qualify for Medicaid Long-Term Care, which can cover the cost of services. Financial eligibility for Medicaid and nursing facility level-of-care eligibility are determined by the state.
If the individual appears to meet the basic criteria and is interested in moving forward, TRU PACE will schedule a meeting with an Enrollment Specialist to begin the enrollment process. If PACE is not the best fit, we will help connect individuals with other community resources that may better meet their needs.
Step 2
The Enrollment visit is scheduled with the potential participant as soon as possible either in your home, at the day center or over the phone. The Enrollment Specialist reviews the PACE program and answer any questions or concerns.
We discuss your medical history, community support, as well as verify your insurance. We complete releases and consents including a Release of Information form to allow TRU PACE medical team to understand your medical needs. If necessary a Medicaid application is also completed. This is a great time to schedule a visit to our day center, have lunch and meet our friendly TRU PACE staff.
Step 3
Members of the TRU PACE Interdisciplinary Team (IDT) meet with the potential participant in their home to complete a pre-enrollment assessment. During this visit, the team learns more about the individual’s health conditions, daily care needs, home environment, and support system to better understand their medical, functional, and social needs.
After the assessment, the TRU PACE Interdisciplinary Team reviews the findings to determine whether the program can safely and appropriately meet the individual’s needs through the PACE model of care. Nursing facility level-of-care eligibility is determined by the State of Colorado.
If the individual meets the program requirements and chooses to move forward, the Enrollment Specialist will contact them to complete the enrollment process. If TRU PACE is unable to accept the individual into the program, the Enrollment Specialist will provide written information about appeal rights and help connect the individual with other community resources that may better meet their needs.
Step 4
Welcome to TRU PACE! The Enrollment Specialist arranges to meet with you to sign paperwork and to complete the enrollment process. We discuss the date that TRU PACE services begin and the specifics of the transition to TRU PACE care. TRU PACE participants may disenroll at any time.
Financial Eligibility
Adults age 55 and older who meet PACE program requirements may be able to enroll in TRU PACE, even if they do not currently have Medicare or Medicaid. Our team can help individuals and families understand their options and guide them through the application process if Medicaid Long-Term Care may be appropriate. Financial eligibility for Medicaid is determined by the State of Colorado.

- Medicaid with a share of cost: Some individuals with Medicaid may be required by the state to contribute a share of cost based on their income.
- Private pay: Individuals who do not qualify for Medicare or Medicaid may be able to enroll in TRU PACE by paying privately for the full cost of the program.

Enrollment FAQ's
Do I have to change my doctor?
TRU PACE includes a team of qualified providers—physicians, nurse practitioners, and physician assistants—who specialize in geriatric medicine and serve as the participant’s primary care providers as part of the PACE care team. Once enrolled, participants receive their primary medical care through the TRU PACE Interdisciplinary Team, which coordinates all medical services.
Participants may continue seeing specialty providers when those providers are part of the TRU PACE network or when services are authorized by the care team.
What about Specialty care and Hospital care?
TRU PACE partners with hospitals and specialty providers throughout our service area to ensure participants have access to a wide range of coordinated care. When specialty care is needed, the TRU PACE Interdisciplinary Team works with trusted providers in our network to support each participant’s medical needs.

Does TRU PACE provide care in my home?”
TRU PACE provides and coordinates in-home care services as part of the participant’s individualized care plan. These services are delivered by trained staff and providers who are part of the TRU PACE care team.
If in-home support is part of your care plan, the TRU PACE Interdisciplinary Team will work with you to arrange the services needed to help you live safely at home.
How often can I come to the TRU PACE Clinic?
Services at the TRU PACE Clinic are provided based on your medical needs, so each participant has an individualized plan of care.
Do you have a Day Center for socialization?
Yes! Our day center is available for you to enjoy the many recreational activities that are provided. TRU PACE day center is open M-F 8am-4:30pm.
Do you help me with meals?
Yes. TRU PACE ensures you have access to food and meal programs, and our home care staff can assist you with grocery shopping, if approved by your IDT.
Do you help me with dental, hearing, or vision?
Yes. Dental, hearing and vision services are provided at TRU PACE.
When can I start getting services?
As soon as you are enrolled in TRU PACE, our Interdisciplinary Team assesses your needs and begin services right away.
