Help When You Need It

As people grow older, their bodies and minds change. Behavioral health at TRU PACE cares for the whole person — body, mind, and emotions – not just physical needs. Many older adults feel stress, sadness, memory changes, or worry, especially when health needs grow. At TRU PACE, you are not alone. The team listens, supports your goals, and helps you feel safe, respected, and understood.

Support is delivered by trained professionals who know how to work with older adults. Services are based on individual needs and provided when the PACE Interdisciplinary Team decides they are needed.

What Behavioral Health May Include

TRU PACE offers a variety of Behavioral Health Services that support older adults who are experiencing anxiety, depression, grief, pain or other symptoms. These services help to improve participant emotional well being, coping skills, and daily life.

Services include talk therapy, one-on-one Counseling  as well as support groups to address issues and improve overall mental health of TRU PACE participants. Behavioral health services vary by person and follow the care plan created by the TRU PACE Interdisciplinary team.

How the Team Supports You

TRU PACE Behavioral Health staff talk with you in simple, clear language. They guide you through what you are feeling and help you understand your choices. If you agree, the team also supports your caregivers and family so everyone feels informed.

The Behavioral Health team works closely with doctors, nurses, therapists, social workers, and activity staff. This means information moves with you, and your care plan stays coordinated as needs change.

How the Team Works With You

An Interdisciplinary Team coordinates counseling, medical care, social work, and therapy under one plan. You have a single point of contact who helps schedule visits, shares updates, and adjusts the plan as needs change. Written information is provided so you can review next steps at home.

If hospital care or a specialist visit is needed, the team helps with records and follow up so care stays connected.

Caregiver Support

Caregivers often feel worried, tired, or unsure how to help. The Behavioral Health team offers:

With your consent, caregivers receive updates and clear next steps. Caregivers receive practical guidance and clear communication. The team can discuss routines at home, safety planning, community resources, and ways to reduce stress for everyone involved.

Behavioral Health support begins with a simple conversation. The team asks about your goals, preferences, daily routines, and what you want help with. Together, the TRU PACE Interdisciplinary Team reviews these needs and creates a plan of care.

There are no guaranteed outcomes. Services may be provided when requested by the participant or their representative and approved by the interdisciplinary team (IDT) as medically necessary and included in the care plan.
