TRU PACE has a healthcare clinic in your service area. We provide free transportation to and from your medical appointments, clinic, and day center.
Our Team is Your Team
Our interdisciplinary team of doctors, physicians, nurses and other practitioners provide hands-on approach, coordinating medical, social and home care services so our participants no longer need to manage their medical care alone. At TRU PACE our team of health professionals specialize in aging and chronic conditions.
If you need to see a specialist, all appointments are handled through your TRU PACE primary care provider and approved by your care team. This coordination ensures that you receive the best possible medical treatment and that all necessary parties are properly informed about your health needs. If you have a medical emergency outside of regular business hours, our nurses are available to support you 24/7.

How Medical Services Work at TRU PACE
As a Participant in TRU PACE you will receive coordinated medical care that is personalized to your needs, all under one roof. When you join TRU PACE, the medical team meets with you to learn about:

- Your goals and preferences
An Interdisciplinary Team creates a personal plan of care. The IDT coordinates doctors, nurses, therapists, and social work as part of your personalized care plan. This plan may include doctor visits, therapy, nursing care, or other services. The IDT reviews goals, explains options in plain language, and helps schedule visits. Transportation is arranged when it is part of the plan, so access to care is simpler.

TRU PACE participants must receive all needed healthcare, including primary care and special physician services (other than emergency services) from TRU PACE contracted providers. TRU PACE participants may be fully and personally liable for costs of any unauthorized out-of-network services.

Getting Started

Qualifications Include;
Age 55 or Older
Eligible for nursing facility level of care as determined by the state
Able to live safely in your community with TRU PACE support
Live in TRU PACE service area
*Qualifying for TRU PACE also includes a functional assessment of your needs for assistance with areas of daily living to help you stay in your home safely for as long as possible and will assist in coordination of your care plan.
