Support to Stay Safe at Home
At TRU PACE, we understand that living independently can sometimes require extra support. Some adults need short-term help while recovering from an illness or injury, while others benefit from ongoing assistance with daily tasks. As part of the TRU PACE model of care, home care may be included when the interdisciplinary team determines it is helpful for supporting safety and independence.
TRU PACE provides coordinated Home Care that supports safety, comfort, and daily routines for adults 55 and older, who meet TRU PACE eligibility requirements. Services are coordinated by one team that knows the person, communicates clearly, and adjusts the plan as needs change.
Home Care takes place where the participant lives. Visits are scheduled to fit the day with the established plan of care. Clear notes are provided for the participant and caregiver so everyone knows what to expect.
What Home Care May Include
- Personal care such as bathing, dressing, and grooming
- Help with meals, light housekeeping, and safe movement
- Nursing visits for monitoring, education or support and teaching when needed
- Coordination with the TRU PACE Clinic and Day Center to keep the care plan aligned
How It Works
Unlike most medical programs, TRU PACE is different. TRU PACE will come to your home to provide needed services to help you maintain your independence and safety. As part of your pre-enrollment assessment, the TRU PACE nursing IDT evaluates whether Home Care could support the participant’s health and daily routines. Based on individual needs, services may include personal care, meal preparation, laundry, medication reminders, light cleaning, and other supports identified in the care plan.
Home Care is provided by trained, certified professionals who communicate regularly with the interdisciplinary team. The TRU PACE team helps schedule visits, explains options in plain language, and shares updates when the plan changes. When needs shift, the team reviews goals and adjusts services. Caregivers can join conversations if the participant wishes.
